Can I use unsanded grout for a smoother look at 3/16"?
No — at this width unsanded shrinkage-cracks down the joint's own centerline. Sanded is structural here, not stylistic. No exceptions.
How much more grout will I need?

The joints won't all be exactly 3/16" — is that failure?
